Filing a request for service is extremely easy to do in our state of the art online RFS Submission and Tracking Center. Just follow the steps below to get your Request for Service Filed.
On the top right side of all site pages is our Login Area. Please choose your member type from the Dropdown Box. You'll need your Member Number and Mobile Device Number.
Once you have logged in click on the File an RFS Icon. All of your account information is already pre-populated for ease of use. All you have to do is post a detailed description of the issue you are having. Read the disclaimer and if you agree, click on Submit Request for Service. Once this is posted it will be assigned an MDPA Representative for review.
An MDPA Representative will forward your request to an Account Manager for processing. A notification is then sent through this Website. Messages from MDPA are updated frequently. Check back at any time by logging in and clicking TRACK RFS.
After your request has been made you can track the request in the same Members Control Panel. Once logged in, just click on the "Track Your RFS" Icon and you'll see up to the minute information about your request.
